Response Mailbox - This is where the notification when a questionnaire is submitted will go.

Started Date - When the Questionnaire was started.

Last Sent - When the Questionnaire was last sent.

Submitted Date - When the Questionnaire was submitted.

Sequence - Is an Admin control. It determines where the Questionnaire will show in the list.
